Scheduling:
This service is run on a call back service. Most calls will be made Tuesday afternoons. When possible, additional return call attempts will be made. When you call please leave your name, the person's name who is to be tested and a contact phone number.
Evaluation time:
Office space is available at Tuesday afternoons. The evaluation will take approximately 2 hours.
Note: Please send the following prior to your appointment or at the latest bring to your appointment:
- Any other testing reports (e.g. neuropsychological, speech & language, occupational therapy, hearing)
- Both listening checklists
- Release of Information form
- Signed HIPPA form
